Since I'm 12 months out of currency for my instrument currency, I'm going to have to take an Instrument Proficiency Check (IPC) really soon. I have a few questions for the check:

1) Is it done by an FAA Inspector or CFI?
2) What are the requirements / procedures for an IPC? Are they the same as the practical test for my instrument rating?
3) If I fail the check, do I lose my instrument rating?

1) CFI.

2) Sooo many details - see here: http://www.faa.gov/pilots/training/media/IPC_Guidance.pdf Basically, you should be able to pass an instrument rating checkride, as well as the oral exam.

3) No! You're just where you were before - not current.

The requirements for the instrument proficiency check are located in front of the Instrument PTS. There is a table there with a column named IPC. Those tasks listed under that column are required to be completed in order for a CFII to sign you off.
